Case Description
I am in Excrutiating Pain on both sides of my mouth on my upper teeth. All of my upper teeth including the front ones are sensitive to both hot and cold. I can barely eat anything. I brush my teeth 2x daily and rinse with Listerine which also causes pain. Parts of my teeth just break off sometimes with no warning. I have had lots of work done in the past, many of my teeth on the bottom were extracted. I would like to avoid that at all cost with my upper teeth as I am only 34 years old.

It sounds like you have chronic problems in relation to some of your upper teeth and couple of those teeth have now turned into acute problems. My recommendation would be for you to see a dentist ASAP, I am afraid that if you dont seek help that it''''ll keep getting worse.

Dear patient:

First, I am sorry to hear about the condition of your teeth and gums, but the good news is that with proper treatment you should be able to restore/replace the teeth to get back to your normal oral health.

Sensitivity and pain you mention above might be due to various reasons. Here are some common reasons:

1. The most common cause of the sensitivity is due to over brushing. If you are brushing your teeth extremely hard with tough bristles may cause some of your teeth to become extremely sensitive. The wrong brushing technique will actually exarcerbate the condition.

2. Poor oral hygiene. This may allow tartar to build up at the gum line, leaching minerals from the tooth.

3. Long-term wear and tear

4. Untreated cavities

5. An old filling with a crack or leak

6. Receding gums that expose the tooth''''s roots. Receding gums often are caused by periodontal diseases or by brushing too hard.

7. Excessive tooth whitening in people who have tooth roots that already are exposed

8. Frequently eating acidic foods or drinking acidic beverages

It sounds like you have a generalized pain condition that needs to be addressed immmediately. The immediate challenge is to pinpoint a particular tooth or area that is causing the generalized pain. You may be experiencing "referred pain" which means the pain is coming from a particular area and traveling to a different region of your mouth. It would be difficult via email to diagnose the exact cause of your pain.

My office does have openings today to accomodate you to relieve you of your discomfort. If you so desire, you are welcome to call my office. My staff will be alerted to address your emergency needs. The fee will probably include a palliative visit and any necessary x-rays. Thank you.
